You only need to be level 95 in the related skill (AND have the related crafting specialization). You do NOT need to be level 95 in combat! My preparations master was able to take the Blighted Mineral Spirits recipe to the npc in Dusk Twins and learn it when she was level 90 combat. (Her smithing, tailoring, and woodcrafting were all level 95.)hi

Only a potions master is able to learn the Blighted Treasure Dram recipe from the npc in Ash White, and they need to be level 95 in cooking.

So I don't know for sure... but maybe you only need to have one skill at lvl 95+ to learn the blighted preps (if you're also a prep master, of course)? I just unlocked Dusk Twins on another toon, and decided to make her a prep master since my "real" prep master won't get to blight for a long time. This toon's smithing is 95, tailoring 93, and woodcrafting 91; but the prep NPC let me learn all 3 of the blighted recipes. I know I need to finish leveling tailoring and woodcrafting before I can actually make the belt & boot preps, but they show up as "known" in the recipe book now.
